Blackheads

They are not dirt: the black is oxidised pigment, which is why scrubbing does not help.

A blackhead is a follicle whose opening is blocked with keratin and sebum. The visible part is open to the air, and in contact with it the contents oxidise and darken. Here is the fact that changes everything else: the black is not dirt. This is why washing more, scrubbing or using harsh cleansers does not remove them, and often makes matters worse by damaging the skin barrier.

Typical sites are the nose, chin and forehead, where sebaceous glands are densest. For the same reason blackheads on the back and shoulders are common too, where the density is just as high and clothing adds occlusion. They are the mildest expression of the acne process and can exist alone, without any inflammation: having blackheads does not mean having acne, even though the starting mechanism is the same.

On home remedies it is worth being explicit, because the damage is frequent. Adhesive strips remove the superficial part but do not resolve the blockage, and leave the opening dilated: the effect lasts a day and then returns. Squeezing with the fingers pushes part of the contents deeper and turns a quiet comedone into an inflamed lesion which, unlike a blackhead, can leave a mark or a scar. The real work is done by active ingredients that regulate cell turnover inside the follicle, not by force applied from outside.

Strips and fingers do more harm than the blackhead

This is what is worth knowing before the next attempt in front of the mirror. An adhesive strip pulls out the superficial plug but leaves the follicle blocked and the opening wider: after a few days you are back where you started, with a more visible pore. Squeezing is worse, because part of the contents is pushed deeper and can trigger inflammation: the blackhead, which is a non-inflammatory lesion leaving no marks, becomes a papule that can. It is not a matter of patience: it is that the action works from the wrong side.

How it shows

  • Small dark dots on the nose, chin and forehead
  • An irregular skin surface to the touch
  • Visibly dilated follicular openings
  • Worsening in the oiliest areas
  • Constant reappearance after mechanical removal
  • Possible association with inflamed papules

How we proceed

  1. 1Dermatological visit: assessment of the picture and its extent
  2. 2Distinction between isolated comedones and true comedonal acne
  3. 3Setting up a routine with appropriate active ingredients
  4. 4Correction of cleansing, often too aggressive
  5. 5Later review: follicular turnover takes weeks

Frequently asked questions

Does washing my face more often help?+

No, and beyond a certain point it makes things worse. The black is not dirt sitting on the surface but oxidised contents inside the follicle: cleansing does not reach it. Washing too much or with degreasing products damages the barrier, and irritated skin often responds by producing more sebum. Two gentle washes a day are enough; the rest is done by active ingredients.

How long before I see a change?+

Weeks, not days, and it is useful to know so as not to give up too early. Effective treatments act on cell turnover inside the follicle, a process with its own biological timescale: improvement is gradual and better appreciated by comparing photographs over time than by looking every morning. In the first weeks some actives can even cause a transient worsening.

Are they the start of acne?+

They share the initial mechanism, but having blackheads does not mean inflammatory acne will develop: many people have comedones for years with nothing else. The step that marks the difference is the appearance of red papules and pustules. If inflamed lesions start appearing alongside the blackheads, that is the moment to be seen rather than continuing with cosmetics.
