Thinning hair on top

Androgenetic alopecia: they do not fall more, they become finer. It is a different process.

Those who notice thinning often do not see more hair in the basin than before. They see the scalp showing through where it did not before, the parting widening, the ponytail thinner. It is the sign of a process different from shedding: here hairs do not leave en masse, they shrink.

The phenomenon is called miniaturisation. With each cycle the sensitive follicle produces a hair slightly finer, shorter and less pigmented than the last, until the shaft becomes almost invisible. Apparent density falls not because follicles are missing, but because those present produce something too fine to cover.

The sites make it recognisable and distinguish it from other pictures: receding temples and crown in men, widening of the central parting in women, with the frontal hairline usually preserved. And here lies the practical consequence: thinning is progressive and time works against it. While the follicle is miniaturising there is room to act; once it is exhausted, there is nothing left to slow. This is why early assessment is not an exaggeration — it is the only moment when choices have an effect.

It is not how many fall: it is what the remaining ones are like

This is the shift in perspective that makes the visit useful. Counting hairs in the brush says little about thinning, because the process does not run through heavier shedding but through progressive fining of those that grow. The comparison that really informs is with a photograph from some years ago, and the examination that measures it is magnified observation of the scalp, where thick and thin shafts are seen side by side in the same area. That unevenness is the sign, not the number.

How it shows

  • Scalp visible where it was not before
  • Central parting progressively widening
  • Hair finer, shorter and lighter than a few years ago
  • Men: receding temples and crown. Women: central thinning
  • Ponytail or bun felt to be thinner for the same amount of hair
  • No itching, redness or scaling of the scalp

How we proceed

  1. 1Trichology visit: sites involved, family history, course over time
  2. 2Magnified scalp observation to measure miniaturisation
  3. 3Exclusion of pictures that mimic thinning and are treated differently
  4. 4Definition of the pathway, bearing in mind the primary goal is to slow it
  5. 5Scheduled follow-up with comparative photographic documentation

Frequently asked questions

If I start treatment, will my hair return as it was?+

The realistic goal is to slow miniaturisation and improve still-active follicles, not to rebuild density where the follicle is exhausted. It is a distinction worth making clear at the outset: someone expecting the head of hair they had ten years ago will be disappointed even when the result is good. Someone who starts knowing they are defending what they have will judge the outcome correctly.

Is it the same in women?+

The underlying mechanism is the same, but the distribution differs: in women thinning is typically central, with the parting widening, and the frontal hairline is preserved. There is also an important practical difference: in women it is worth excluding other contributing factors, because more often than in men the picture is mixed, and correcting that contribution changes the result.

How early is early enough?+

The useful moment is when thinning is noticeable but not yet marked — exactly the moment people tend to postpone because «it is not that visible». The reasoning should be reversed: you act while follicles are miniaturising, not once they have stopped. An early first assessment, even just to document the starting point, is worth more than one made when the decision is already forced.
