Your concern

Constantly sweaty hands

Palmar hyperhidrosis: the form that most affects work and shaking hands.

Among the sites of hyperhidrosis, the palms weigh most in daily life, and not for cosmetic reasons: a wet hand conditions what you do with your hands. These are sweaty hands even at rest, without heat and without exertion, not hands that get damp before an exam. Writing, using a phone or a mouse, handling paper, playing an instrument, shaking someone's hand. It is a symptom many people manage silently for years, avoiding situations, without knowing it is a condition with a name.

The palmar form is typically primary: it begins at a young age, is symmetrical, runs in families and, a consistent detail, stops at night. The sweat glands are normal; what is not normal is the intensity of the nerve signal reaching them. Emotion triggers it but does not cause it: this is the misunderstanding that leads many people to see it as a matter of temperament.

It is worth saying clearly that it can be treated, because that is why this page exists. After first-line topical measures, botulinum toxin at the indicated sites is a concrete option for the palms too. The pathway begins, however, with the same distinction as hyperhidrosis in general: primary or secondary form, because they are not approached in the same way.

It is not shyness: it is a condition with a treatment

This is the information many patients receive for the first time at a visit, after years of managing the problem privately. Primary palmar hyperhidrosis is not an expression of temperament: emotions trigger it as they trigger blushing in those who blush easily, but the predisposition exists independently, and indeed the sweating appears in neutral situations and at rest too. Recognising it as a condition shifts the conversation from «how to hide it» to «what can be done», which is a different conversation.

How it shows

  • Palm sweating out of proportion to temperature and activity
  • Visibly wet hands, with droplets at rest
  • Difficulty writing, using devices, handling paper
  • Symmetrical distribution on both hands
  • Onset at a young age, often with cases in the family
  • Absence of the symptom during sleep

How we proceed

  1. 1Visit: sites involved, age of onset, night-time pattern, family history
  2. 2Distinction between primary and secondary forms
  3. 3Topical measures as a first level, where sufficient
  4. 4Botulinum toxin at the indicated sites when they are not enough
  5. 5Follow-up to assess duration of effect and maintenance timing

Frequently asked questions

Is treatment on the palms painful?+

The palm is a more sensitive site than the underarm, and this should be said beforehand rather than after: measures are used to make the procedure tolerable, agreed at the visit. It is one of the aspects discussed with the patient before deciding, along with expected duration and maintenance timing, because at this site advance information is part of the choice.

Will I lose strength in my hands?+

It is the commonest concern, and a legitimate one because the same molecule acts on muscles when used for other indications. In treating hyperhidrosis the target is the signal to the sweat glands and the injections are superficial, but on the palm the proximity of the hand muscles is taken into account in the technique. It is exactly one of the things to discuss at the visit before proceeding.

Are there things to try first?+

Yes, and the pathway starts there: topical aluminium-salt preparations exist with specific concentrations and instructions, which in a proportion of patients are sufficient. They must be used correctly — when they are applied matters more than you would think — which is one reason it is worth having them prescribed rather than bought at random.
